Job description: The Hospice RN will be responsible for initial, comprehensive and ongoing assessments, initiating the plan of care and making revisions as the patient's needs change, counseling patient and families in meeting nursing needs, communicating with the physician regarding patient's needs and reporting any changes in the patient's condition, obtaining and receiving physician's orders as required, and counseling field staff when the need arises.
