Position: Wellness Program Worker
Position Type: Part-Time, Permanent
Hours of Work: 21 hours per week, In-Person Position.
Compensation: $19.96 - $24.95 Hourly
Reports to: Collaborative Practice and Care Services Manager

Position Summary:
This position supports seniors and other clients living with mental health conditions, addictions, and/or substance use concerns through group facilitation, educational workshops, and individualized support aligned with care plans. The Wellness Support Worker assists Care Managers by connecting clients with resources, accompanying clients to appointments and activities as appropriate, monitoring and reporting client well-being, completing documentation, supporting outreach and administrative tasks, and promoting client and staff safety in all aspects of the role.

Responsibilities:
  • Group and Workshops facilitation, including organizing and leading social groups for seniors living with mental health and/or substance use concerns to help combat isolation.
  • Plan and conduct workshops regarding seniors' mental health and substance use issues.
  • Develop educational materials and presentations to raise awareness, understanding, and reduce stigma.
  • Conduct research and prepare materials for Care Managers, including PowerPoint slides and activities for group meetings.
  • Assist in creating marketing materials to promote agency programs and services (e.g. flyers, brochures).
  • Assist Care Managers in carrying out care plans and recommendations, which may include, but are not limited to: Encouraging and accompanying client(s) to attend medical appointments, recreational activities and/or running personal errands.
  • Connect with and set up appropriate resources as set out in the care plan
  • Completes necessary documentation in a timely manner
  • Ensures client and staff safety in all aspects of responsibilities
Education:
  • Post-secondary education in mental health, addictions, social services, or a related field is required
  • Valid First Aid and CPR certificate an asset
Skills & Experience:
  • A minimum of four to six months ( 4 to 6) experience working with mental health and addiction or substance use issues. Placement experience is acceptable.
  • A passion for working with seniors and individuals with mental health and addiction or substance use issues.
  • Strong organizational and communication skills.
  • Ability to work independently and as part of a team.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Ability to maintain confidentiality
  • Knowledge and understanding to work in a culturally diverse environment
Working Conditions:
  • Required to conduct home visits with clients and caregivers; working alone in the home
  • Regular interruptions to support clients’ needs and crises
  • Regular exposure to computer terminal

  • A valid driver’s license, and access to a vehicle are required for home visits.
